A doctor's referral is required
for Bone Densitometry or to participate in our program.
Program Referral
Process
We must have the following information
from your doctor before we can book an appointment. This information can
be faxed to us from your doctor's office:

referring doctor's name and
phone number
-
your name and address
-
home and business phone numbers
-
Health Card number
-
date of birth
- copy of Bone Density results
(unless the test was done at Women's College)
